Saline implants, on the other hand, are filled with sterile saltwater solution. However, they may feel firmer than silicone implants and tend to have a less natural appearance. All implant shells are made out of medical-grade silicone. The two primary types of implant shells are smooth and textured. Smooth implants have a soft, smooth outer surface and tend to move more naturally within the breast pocket. They are often chosen for a more natural look and feel. On the other hand, textured implants have a slightly rough surface designed to adhere to the surrounding tissue. There have been safety concerns linked to textured implants which is why these implants are no longer available and offered at our practice.


Textured Implants

Textured implants have become less popular in recent years due to concerns over a condition known as Breast Implant-Associated Anaplastic Large Cell Lymphoma (BIA-ALCL). This rare form of cancer has been associated with textured implants, particularly those with a higher degree of surface texturing. While BIA-ALCL is still relatively rare, the medical community and regulatory agencies have taken these concerns seriously, leading to a decline in the use of textured implants.

It’s important to note that the risk of BIA-ALCL is low, and many women with textured implants have not experienced any problems. Nonetheless, the concerns surrounding textured implants have led to increased awareness and scrutiny in the medical field, and alternative options like smooth-surface implants or implant technologies with lower-profile texturing are often considered to minimize potential risks while still achieving desired aesthetic outcomes.


Choosing Your Incision Site

Breast augmentation offers several options for incision placement, each with its advantages and considerations. The most common incision techniques performed at our practice include inframammary, transaxillary, and periareolar incisiosn. Each incision type has its advantages and potential drawbacks, and at your consultation we will help you determine which option aligns best with your goals and anatomy.


Inframmamary Incision

Inframmamary Incision

The inframmamary incision is the most common incision for breast implant placement. This incision is placed in the natural breast crease. This approach provides direct access, precise implant placement, and minimal interference with breast tissue. If a patient does not have a natural breast crease, this approach may not be ideal.


Transaxillary Incision

The transaxillary incision allows your surgeon to place your implants through the armpit. This results in no visible scars on the breasts. The incision is made in one of the natural creases in your armpit and is virtually undetectable when the arms are down. Like the inframmamary approach, this approach involves minimal interference with the breast tissue. An endoscope, lighted camera, is utilized to precisely place the implants in the correct position. Utilizing a transaxillary incision is considered a more technically challenging approach, however our surgeons specialize in this technique in order to yield equally consistent results.


Periareolar Incision

Periareolar breast augmentation involves making an incision along the lower edge of the areola, the dark-colored skin surrounding the nipple. This incision is the most concealed in comparison to other approaches due to the natural color change between the breast and areolar skin. This approach, however, may impact nipple sensitivity due to the scar location and interference with the breast tissue. This approach is utilized most commonly when a breast augmentation is combined with a breast lift, or mastopexy, since an incision is required around the areola.

There are two methods for placement of your breast implants: sub-muscular/pectoral and sub-glandular/facial approaches.

The choice between submuscular and subfascial placement depends on various factors, including the patient’s anatomy, existing breast tissue, implant type, and aesthetic goals. During your consultation, your surgeon will help you determine which approach is better for you.

This approach involves placing your implants partially under your pectoralis major muscle. This is the most common approach in our practice. This placement results in a more natural appearance and a smoother transition from the chest to the breast. As a result, you are less likely to see your implant which can can be a concern for individuals with little to no natural breast tissue. Additionally, placement of the implant underneath your muscle has been seen to decrease risks of capsular contracture. Your pectoralis major muscle also separates the implant from the natural bacteria in your breast, which can result in a decreased infection risk. This approach also enhances visibility for mammograms.


Sub-Glandular (Sub-Facial) Implant Placement

Sub-Glandular (Sub-Facial) Implant Placement

This approach involves placement of your implants behind your breast (glandular) tissue and above your pectoralis major muscle. It typically involves a shorter recovery period with less post-operative discomfort compared to submuscular placement. There is an increased risk of visible rippling or wrinkling of the implants, particularly in individuals with minimal natural breast tissue. This approach has been shown to increase the risk of capsular contracture.

breast lift, also called a mastopexy, may be needed in combination with breast augmentation to correct breast ptosis. Breast ptosis, commonly referred to as breast sagging or drooping, is a condition characterized by the downward displacement of the breast tissue and nipple-areola complex. It occurs when the supporting structures within the breast, such as ligaments and skin, lose elasticity and firmness, leading to a loss of breast shape and elevation. If your nipples need to be repositioned, you will likely need a breast lift at the same time as your implant placement.

During the initial days after the procedure, patients may experience some discomfort, swelling, and bruising in the chest area. It is completely normal to feel a “tightness” sensation in the chest area. Pain medications will be prescribed by the surgeon can help alleviate discomfort. However, it is highly encourage to utilize over the counter pain medication to manage the discomfort.

Rest and limited physical activity are essential during the first week to minimize strain on the surgical site. Patients are typically advised to wear a supportive surgical bra for several weeks to aid in proper healing and minimize swelling. As the weeks progress, most individuals can gradually resume light activities, but strenuous exercise and heavy lifting are generally restricted for a few weeks to ensure the incisions heal properly.

Full recovery and final results can take several months, during which the breasts will settle into their desired position, and any residual swelling will subside. Your breasts will appear in a higher position at first due to swelling. Your scars will initially be raised and pigmented, but will flatten and fade with time (typically 6-12 months following your surgery).

Each patient’s recovery timeline may vary, and it’s important to follow your post-operative instructions diligently to achieve the best possible outcome as well as following up with your surgeon regularly.

Significant complications from breast augmentation are infrequent. However, we will make sure you understand the risks and possible complications of breast augmentation. Some of  the risks include the following:

  • Infection (you are given antibiotics through your IV at the time of surgery)
  • Capsular contracture (tightening of your breast capsule which is the scar tissue that surrounds your implants)
  • Loss or change of nipple sensation (uncommon, however can occur when utilizing a periarolar incision)
  • Implant Rupture/Leak (will require removal or replacement)

Like any surgery, undergoing a breast augmentation poses a risk of bleeding and adverse reaction to anesthesia. A thorough understanding of the potential risks and benefits can help you make an informed decision about breast augmentation.

  • Although breast implants are engineered to be a lifetime device, they aren’t guaranteed to last a lifetime. Aging, weight gain/loss, breast feeding, and other macros may change the appearance of your breasts with time.
  • Be sure to inform your mammogram technician of your breast implants. Placement of breast implants in the sub pectoral approach allows for easier mammography imaging.
  • Breast augmentation may alter a woman’s ability to breast feed. Usually, women are able to successfully breastfeed after breast augmentation.
  • If you decide to have your breast implants removed, there is a chance you will need additional surgery such as a breast lift.

How long will my surgery take?

A breast augmentation surgery typically lasts about 1.5 hours. However, this may vary from person to person.

If you are looking to increase the overall volume of your breasts, then breast implants will help you achieve that goal. A breast lift will only remove excess skin and reposition your nipple areolar complex without changing the volume of your breasts.

We encourage walking and mobilization the day after surgery. You may slowly start implementing regular physical activity 1-2 weeks after surgery.

Yes, you can have a breast augmentation utilizing your own fat through a procedure known as autologous fat transfer or fat grafting. This approach involves liposuction to harvest fat from donor areas of your body (such as the abdomen, thighs, or hips) and then purifying the collected fat before injecting it into the breasts to achieve the desired volume and contour. A final size cannot be 100% guaranteed as fat grafting is more unpredictable.

If your breast implants are not ruptured or causing you any issues, then there is no medical reason to replace them.
